Job Description

The Care Coordination Analyst is responsible for designing metrics reports and performing analytical analysis to assist and facilitate decision support for the Care Coordination Manager and Benefis Health System Leadership in achieving the overall goals, values, and strategic endeavors. The analyses include, but are not limited to, statistical analysis, financial analysis, productivity analysis, and other data analysis from multiple databases used in utilization review, discharge planning, and concurrent documentation operational processes. The Care Coordination Analyst oversees production and delivery of vital reporting statistics that support management in key decision making and in the oversight and leadership of the Care Coordination Department of Benefis Health System. In addition the analyst will lead and/or participate in various committees and facilitate as deemed necessary by the Care Coordination Manager as they relate to care coordination, transition planning, and metrics development and analysis.

DUTIES AND RESPONSIBILITIES: Develops key metrics reports and assists in data analysis for the Care Coordination department and reviews with department Manager on routine basis. Data analysis will include but is not limited to the following: Volume of Observation/Inpatient cases; Extended recovery by payer; by Provider, by area; Length of stay (LOS) by payer, by Provider, by area; Care Coordination scorecards, and other metric reports as indicated. Track and trend patterns of non-compliance for payer sources and Providers and performs other analytic duties at the direction of the Care Coordination Manager. Prepares and presents data on productivity, inpatient and observation reports on a daily basis and as requested by Care Coordination Manager. Prepares day end Payer report reconciliation. Develops and maintains reporting metrics and implements updates and changes to Care Coordination databases. Coordinates and Conducts staff training on program software updates, data documentation, performance and outcome measurement requirements. Maintains Payer contract database and apprises Care Coordination manager and staff of any new changes affecting facility reimbursement. Knowledgeable of regulatory compliance requirements for department and integrates requirements into data reporting methods that measure compliance with current regulations. Conducts routine chart audits for Utilization Review, Transition planning, and Concurrent Documentation compliance with regulations. Records, complies, and tracks and trends audit related data. Utilizes information to facilitate improvement opportunities of Care Coordination department with Manager. Assists with planning and implementation of Care Coordination department projects. Completes data entry in Care Coordination databases at direction of Utilization Review staff and Care Coordination Manager. Works collaboratively with UR staff to complete pre-certification authorizations as required and prepares reports as necessary. Performs Midas+ operational support for the System and works in collaboration with Information Technology for Midas+ technical support. Operational support includes but not limited to the following: training users, maintenance of modules, dictionaries, and work lists, manage Reportrack and Smartrack, customize user support, maintain system templates and indicators; troubleshoot for operational issues, collaborate and participate in decisions related to Midas+. Develops and maintains Midas+ reports for Benefis Health System as requested by Managers and a variety of committees. Leads and/or Participates in the following committees but not limited to: Utilization Review Committee, Complex Case Management, UR and TP huddles, Quality and Safety, Readmission Committee, and Revenue Cycle Committee. Develops employee schedules in collaboration with Care Coordination Manager and staff. Reviews employee payroll for approval by Care Coordination Manager. Works with Care Coordination Manager to achieve appropriate staffing levels required when employees are on PTO or Holidays. Assists Care Coordination and Transition Planning staff with forms completion as it relates to patient transfers, etc. Acts as resource to staff related to work processes and requirements. Performs QA audits for Utilization Review and Transition Planning. Works in collaboration with Clinical Denials Specialist to perform and complete additional QA audits as necessary. Works in collaboration with the Lead Concurrent Documentation Specialist in developing, implementing, and monitoring CDI metrics. Other duties as assigned by department manager. Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations. Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.

Qualifications

Bachelor’s degree preferred Associate degree accepted in related field.

Two to three years of clinical experience in a health care setting.

Two to three years of data analytics preferred.
